Hospice Care at Home

We know that many of our patients want to receive care in their own homes and our Community Nursing Teams are here to do just that, all across Buckinghamshire and its borders.

Our team of Clinical Nurse Specialists are experienced nurses who have specialist training in palliative care and will work alongside other health and social care professionals to ensure you and your loved ones get all the support you need. This includes symptom control, pain management, advice on difficult emotional and spiritual issues and understanding what is happening.

The team see people at different stages at their illness from newly diagnosed to end of life care

Our Discharge Co-ordinator in the Hospice who is dedicated to ensuring our patients have the support and equipment in place to return home after a hospital or hospice stays.

End of Life Care at Home

FNH@Home is a dedicated team of Nurses and Healthcare Assistants who care for patients in their final weeks and days where they live.

We believe that in order for patients to die at home, their support network needs to have the confidence and belief that they can enable this to happen. Our FNH@Home team strives to give you the support you need to do this.

They are there to ensure that patients can receive the same level of care and medical attention that they would receive in the In-Patient Unit at the Hospice, as long as their symptoms and condition allow that care to be provided in their home.

The FNH@Home team works alongside professional colleagues from Adult Community Healthcare Teams, FNH Community Clinical Nurse Specialists; GPs; other Specialist Clinical Nurse Specialists etc. to support patients and families.

How can I be referred to Florence Nightingale Hospice

Most of our patients are referred to us by their GP, District Nurse, Community Palliative Care Nurse, or a hospital consultant.  If you feel you would benefit from hospice care at home, please talk to your healthcare provider.

There is no cost for patients to receive care at home from Florence Nightingale  Hospice.  The care is delivered through a partnership between Buckinghamshire Healthcare NHS Trust and Florence Nightingale Hospice Charity.
