Make a Referral

The information below is for healthcare professionals only.

If you are a patient or their family or carer and would like to access our services, please speak to a healthcare professional involved in your care, for example, your GP or district nurse so that they can submit a referral on your behalf.

The Hospice’s catchment area is Buckinghamshire plus some parts of Oxfordshire (around Thame) and south Bedfordshire (around Leighton Buzzard).  Patients must be registered with a Bucks Healthcare GP.

Referrals are prioritised according to the complexity of the problems presented.

Referrals to all services must be made using the Buckinghamshire Palliative Care Referral Form and returned to buc-tr.fnhspa@nhs.net

If the request for an In-Patient bed is urgent, please ring 01296 332600 and speak to the ward doctor.
